General comments

Let op: in verschillende bronnen worden de doseringsadviezen onderverdeeld in Inductie en Onderhoud.

Hiermee wordt NIET bedoeld: Oplaaddosis en Onderhoudsdosering;

maar WEL: Inductie = Therapeutische dosering en Onderhoud = Profylaxe 

Behandelduur

Bij milde CMV colitis behandeling 14 dagen. In andere gevallen 21 dagen of langer op geleide van klinische respons.

Behandelen totdat klachten weg zijn en CMV PCR in twee opeenvolgende plasma-monsters negatief is (aangeraden wordt om 1 - 2 week te monitoren). NB: plasma is niet altijd positief bij gastro-intestinale CMV ziekte. Klinische respons is dan leidend; overweeg scopie om therapierespons te evalueren.

Valganciclovir (2dd 900 mg po) kan ook gebruikt worden in plaats van ganciclovir i.v.

Orale therapie wordt NIET AANGERADEN bij: 

- levensbedreigende CMV ziekte of

- indien er vermoeden is op verminderde absorbtie van valganciclovir of

- bij vermoeden van therapieontrouw

Indien mogelijk immuunsuppressiva afbouwen (SOT), of immuunstatus verbeteren (start cART bij AIDS).
