amoxicilline iv loading dose 2g, 12g continuous per 24 hours 6 weeks

ceftriaxon iv 2000mg 2dd 6 weeks

Indications: Resistentie tegen amoxicilline of penicillineallergie, én GEEN high level gentamicine resistentie [HLAR]

vancomycine iv loading dose 20mg/kg, 30mg/kg continuous per 24 hours 6 weeks

gentamicine iv 3mg/kg 1dd 4 weeks

Remarks:

Vancomycine continue-dosering op geleide van TDM, zie Opmerkingen

 

Indien HLAR voor gentamicine wordt vancomycine monotherapie gegeven

Vancomycine: na de oplaaddosering wordt direct vervolgd met continue infusie/24 uur

Overleg met endocarditis-team (internist-infectioloog of arts-microbioloog)

Advies opnieuw gepubliceerd na beoordeling in kader van revisie SWABID UMCG in opdracht van CAM 1-8-2022
